That is a very high number for acute gastroenteritis but it is still possible. The differential dx between acute gastroenteritis and acute pancreatitis may be hard when the symptoms of lower GI tract has not occurred yet. However, when you have diarrhea subsequently, acute gastroenteritis is more likely. Practically, CT of the abdomen is often done when lipase is that high to rule out pancreatitis
