It is correct to stop hctz and replace it with spironolactone. However, with your medical history I assume you are also taking blood pressure medication which belongs to ACE inhibitor group such as lisinopril, ramipril, etc. or ARB group such as losartan, valsartan, etc. Combination use of spironolactone + either ACEi and ARB drug can raise potassium (K) in CKD stage 3 if you do not limit K intake
