Cystitis, especially the hemorragic type, can cause 3 + blood and 1+ leukocyte, more important are your symptoms. If you have frequency and painful voiding (dysuria), bladder infection (cystitis) is likely, especially if nitrite is also positive (negative nitrite does not rule out infection though) and urine culture can confirm that. Your doctor may treat it empirically without urine culture.

Could no understand your question. Up to 3rbc/hpf are not considered abnormal and are not indicative of UTI. If you have no symptoms you need no treatment.
