A perforated ulcer will cause acute abdomen and emergent surgery is required, PPI won't fix it. If you think you have a perforated peptic ulcer you need to go to the hospital ASAP. However, a benign peptic ulcer can be treated with PPI but it can recur if you have H. pylori which needs to be treated differently.
