Can happen: Some of the buccal bone can break during an extraction. There are even times that the roots of the tooth are not even in bone on the buccal surface.

Technique sensitive: Buccal plate or bone wall's thickness varies from tooth to tooth in same mouth and in different persons as well. But even in thinnest situation with correct technique can be preserved in many cases. You need to see a dentist with lots of experience in immediate implant placement.
