On lifelong warfarin & known to have an anaphylactic to iv vit k. now doctor want to do a drug provocation test for the oral version (she think i might have a reaction to the iv preparation, but not the vit k itself). is it necessary? worth of risk?

If what is meant by a "drug provocation test" is a skin test with just the IV prep, it may be worthwhile for a diagnosis, but unnecessary, since a true anaphylactic reaction may still occur, especially given your history.

Sounds reasonable, your doc is trying to secure a way you would be able to still receive vit K whenever needed to reverse a supratherapeutic INR. In emergencies, such as bleeding, plasma products are being used to reverse anticoagulant effect of warfarin.
