A family member has a myeloproliferative disorder. he does have a good hematologist and is stable. however, generally speaking, can an acute transformation take place over a few months, a few weeks or even a few days? we need to be prepared?

Most people with this order live years with few complications. If there's myelofibrosis where scarring of the marrow occurs, life expectancy falls to several years. Some people with myeloproliferative disorder morph into acute leukemia, a dire situation. The hematologist can advise you best.

Yes, acute transformation is a well described complication of chronic myeloproliferative disorders, luckily relatively rare, but with life terminating potential. Definitely now would be the right time to explore with hematologist and your relative possibilities and ways of recognizing such a complication in clinical context.
