Try hypomethylating: CMML is common form of overlap syndrome, can be treated with hypomethylating agent (VADAZA) as MDS. Efficay? If acute transformation to acute leukemia (AML), then should treat leukemia. If pt is older and not a candidate for intense induction, hypomethylating is also an option. For other acute transformation, treat them accordingly. Sometime, just supportive care or clinical trial consideration.
