HDL is the "scavenger" of Triglycerides, which together with free fatty acids (FFA's) are what damage the lining of blood vessels leading to plaque formation, so yes, they should be treated with an agent specific to them, like fenofibrate. Insulin Resistance causes the elev. Trig's, so agents for DM that treat IR like Metformin and Sitagliptin are good choices.
