After 3 days of Macrobid but dysuria persists and now your back is hurting, antibiotic should be changed. If you develops fever, you may need to go to urgent care for IV antibiotic(s).

I believe that a prompt urine culture and antibiotic sensitivity should also be performed to insure that new antibiotic is effective.
