Upper urinary tract infection (pyelonephritis) is not as common as lower urinary tract infection (cystitis) but it is not rare in women. If CT scan with contrast did not show any abnormal finding such as kidney stone or evidence of urine back up to the kidney I would not investigate further. However, if it recurs in the next 12 months, cystoscopy is indicated to rule out bladder lesion.
