A member asked:

My dad has a stent that has fractured and it was placed in his duodenol valve at the time, the area is now blocked (cancer) & he is on tpn - we know this cant be long term & the docs advise he could be weak for surgery, what can be done?

If your father's cancer has progressed to the point where TPN is being administered, adequate nutrition should be provided via the TPN. Cancer which has progressed will gradually cause significant nutritional deficiencies, but surgery to replace the stent in the duodenum will not appreciably change the clinical situation at this point.

A goals of care discussion with the treating team, including oncology and surgery should take place. In case no meaningful intervention is offered, palliative care and possibly hospice if available should get involved with goals being symptom control/ comfort oriented care.
