Orthodontist: Your son needs a consult with a dentist/orthodontist. They can take xrays and see the relationship of all the teeth/molars in the area. Based on this, the dentist/orthodontist may perform an extraction or recommend orthodontic appliances (braces) to move the molar to the right position.

Orthodontist: Standard of Care is that all children see an Orthodontist for a (free) screening examination by their 7th b'day. As an Orthodontist is a dentofacial-facial growth/development expert there were interceptive procedures that could have helped. Your son's malocclusion will only worsen with time. Ask your General Dentist to recommend the Orthodontist he would choose to treat his/her family.
