Many reasons: Non visualization with acute pain can be acute cholecystitis or acalculous cystitis with cystic duct (duct from gb obstruction. If symptoms chronic, then could be related to cystic duct obstruction and /or chronic gb disease, sometimes prolonged fasting, non fasting for sufficient time, and some pain medications can interfere with gall bladder visualization.

GB/cystic duct prob.: In a normal hida scan, radioactive material flows from the liver through the cystic duct into the gallbladder and also through the common bile duct into the intestine. When the gallbladder is not seen, it means either the cystic duct is plugged or the gallbladder is dysfunctional/full of stones or sludge. A gallbladder ultrasound would help.
