Could simple ovarian cyst be mistaken for complex ovarian cyst during transvaginal ultrasound? if it is 4cm and two years ago it was nothing there.

Yes, but should : be followed to be sure it resolves, some simple cysts also have small hemorrhagic component, if increasing pain to needs recheck. If calcium, bony or tooth like areas, could be a benign dermoid (teratoma).
