Some basics: This is usually from the hips. If he sits on his feet it should be with crossed legs, not like a W . He should never sleep with his legs folded under him. Stitching his bed garments from crotch down gradually to stop this, making him sleep straight legged. I would do this for 6 mo to see if he improves. An ortho doc could consult but not much they would do otherwise at that age.
