Maybe rheumatologica: Go to see a good doc and give him/her a thorough history and physical exam and then maybe some laboratory tests to figure out what is going on. Good luck to you.

Unbearable= worrying: Your description of the pain as unbearable doesn't really sound like just simple rheumatic pain. It sounds more like a nerve-related or a vascular (blood supply) issue. However both of these would be less likely to be present only when you're feeling cold. A 400 character prevents us from getting to the bottom of this. As the others say, you need to be physically assessed.
