Offloading: If the MRI is negative or even before that you can take the pressure off of the sesamoids with an orthotic or padding in your shoes, this should help with the pain. Your doctor could modify your shoe insert to help with this. Also you should not go barefoot, as this puts more pressure on the area. and avoid heeled shoes or shoes with a very narrow toe box.

For more severe cases of sesamoiditis, I will put patients in a walking cast with an extended toe plate for 1 month, and sometimes follow this up with a boot for another one month. Finally, transition back to supportive shoes with custom orthotics that unload the sesamoids.
