What can be the clinical medical reason behind a person in hospice cancer care no longer receiving a blood thinner as a daily maintenance medicine?

Blood thinners: There are a number of "blood thinners", and the risks of these varies, and the risks of taking can be increased by some medical conditions. If for example the "blood thinner" is for a condition where a clot might develop in 6% of people per year, and the person is frail and at high risk of falling, the physician may believe the risk of taking the blood thinner outweighs the benefit.

Goals of care: Goals of care for patients in hospice are geared towards symptom control. Blood thinners are considered high risk, due to the risk for bleeding and unless it is believed that stopping them may create uncontrollable symptoms, they no longer serve any meaningful purpose.
