Multiple Sclerosis: If the mr only show a single spot in the cerebellum then ms would be of a very suspicion as to diagnosis - further workup by a neurologist would be helpful here.

Nonspecific: Your symptoms could be due to many conditions, as excessive daytime sleepiness indicates a sleep disorder or even fatigue state. Chronic pain might be fibromyalgia or a peripheral neuropathy. Problems with concentration could be associated with any of the above, but might be due to thyroid or B vitamin issues. Brain MRI lesion is also nonspecific, perhaps a prior stroke? Get second opinion.
