I think it would: make more sense to buy and wear a medical alert bracelet or necklace. Emergency personnel and health care providers are not trained to look for tattoos but they are trained to look for the bracelets and necklaces. If you really want a tattoo choose something more decorative, it is safe on Xarelto if done in a reputable sterile tattoo shop. You might have a little more bruising that's all.
