Which tooth?: Some pain can be referred. You may have a sinus condition, could be an impacted tooth, could even be a cracked tooth. Have your dentist take another look or see an endodontist.

Non-odontogenic: "toothache" is a referred pain from other structures: sinus, ear, muscle, heart and nerve (Trigeminal Neuralgia). See orofacial pain specialist for differential diagnosis and treatment.

Depends: depends on what has been done in that area or if you have any other issues (grinding etc). It could easily be a earache over a toothache and vice versa. To rule out a true ear issue I would see my MD or ENT but if you still think it is tooth related and your doc sees nothing then I would get a 2nd opinion. No worries
