Rotator cuff injury: You may have a rotator cuff injury- anything from a chronic bursitis 9inflamed/irritated bursa overriding the rotator cuff), or a partial to small full thickness tear of the actual rotator cuff tendon. Seek orthopaedic surgeon evaluation. X-rays and perhaps an MRI, if indicated, along with a thorough history and examination should lead to a diagnosis and treatment plan.
