I have a very sharp pain when i breathe in my right shoulder. it's worse when i lay down or bend forward. it has made carrying my bag a challenge. i did a chest x-ray and it was normal, 7sessions of physiotherapy but still nothing. what could it be?

Neck or up back: If you carry a heavy back pack this can cause problem in neck or back that may not show on x-ray. May need to have structural exam or see a Sport Medicine Doctor
