Not necessarily: You are likely describing an inguinal hernia, with bowel and fat contents herniating through abdomen into inguinal canal and sometimes into the scrotum. You may feel a bulge when performing heavy lifting, but typically a small hernia may not be painful at all. Extreme pain would only happen if the hernia twists on itself, which could cause bowel obstruction and a surgical emergency.
