First get her: to an orthopedic surgeon to make sure that cast is not too tight. Then get some advice on how to handle pain in a child with fracture.

When?: If the cast was just put on in the last 24 hours, contact your orthopedist immediately as this may be serious. If it's been on longer, the cast may simply need to be changed. There is nothing she can do - she needs help.

Call orthopedic: doctor asap. Elevate and ice the area and keep off foot till you talk to doctor. If worse to ER.
