The money from a...: Patient's payment is deposited into a bank; and used to pay the dental clinic's supplies, equipment, salaries, rent, etc. (as in every endeavor in our community). The question is whether the implants are under warranty, and for how long. Perhaps, if patients pay high prices, there is enough money to cover warranty work. If patients pay low prices, there is inadequate money to cover warranty work.

Depends: Go back to the Specialist and ask the question. In the absence of neglect , most surgeons will replace the implant at no cost. It is as easy as asking the question. Typically this is discussed before the treatment is agreed upon. If you have been compliant, most will replace. Good Luck. Go Ask.

Guarantee: You're asking for a guarantee. For specifics regarding your personal care, ask the specialist who is treating you what they will do if your implants fail.

New implant: Some failure happens despite the best efforts of the dentist and the patient. Your surgeon will re-evaluate and often place another implant (free of charge).

Depends: If the dentist is ethical they will evaluate the area, the implant and the work that was done for free. If it is determined that it was their fault then I would think that they would do their best to rectify the situation or give the patients money back.
