Twist and shout: Of course the safest thing to do if you are concerned about the severity is to have the ankle imaged. X-ray most likely. The immediate treatment should be ice compression and elevation. I would expect pain to some extent but after a day or so the pain should be reducing. Try to limit the amount of unnecessary pressure to the foot and see how it goes. Some times a wrap helps.

Yes: Particularly if the pain persists for more than 6 weeks. Most ankle "sprains" heal within 2-6 weeks on their own but if pain and difficultly walking, running or on stairs. It would be best to get evaluated.
