Could be: I would get a clinical exam by a foot/ankle doc. It sounds like an MRI would be in order as well.

More than a sprain: A sprained ankle gets better in terms of the acute pain usually in 2-6 weeks. After that it may take upt to 2-3 months more to rehabilitate it. Pain on weightbearing 2 years out is cause for concern and should be evaluated by a doctor. After an examination a plain xray followed by other special tests may be in order (mri, ct scan).

Have it evaluated: Twisting ones’ ankle can lead to a sprained ankle. Often times, this may result in ligament or tendon injuries which may lead to ankle bruising, swelling, pain, and possibly instability. An x-ray of the foot and ankle will be able to assist in differentiating a sprain form a fracture. You may need a ct or an MRI to look for other types of soft tissue or cartilaginous injuries.

Post-traumatic : Many times asked an "innocent" ankle sprain other post-traumatic manifestations will occur. These include bone spur formation. These spurs cause anterior ankle impingement with greater degrees of dorsiflexion. A simple ankle x-ray would tell you if this problem exists. Sometimes these spurs are subtle, but in a mechanically bad location, so they cause a big functional problem.
