True polyp yes: 90%of polyps are actually a stone stuck to the gallbladder wall. So true polyp is rare,but if it is a true polyp ,it has to be removed.because most true GI polyps could turn to cancer. So depend on your symptoms,my advice removed the gallbladder.

Need to be concerned: Polyp 2 cm or over tend to turn into cancers , require close observation , if size is increasing surgical removal of gall bladder is the best option.

Yes: Latest data suggests that polyp >8mm should probably trigger gallbladder removal, used to be 10mm trigger
