Is a fever of 103.6 normal for a kidney infection. she's on day 1.5 of 5 for nitrofurantoin. acetaminophen controls the fever still?

Yes and no: fever is not uncommon with pyelonephritis. generally, I don't believe that five days of an anabiotic such as nitrofurantoin is sufficient to treat a kidney infection.

IV Abx: A fever this high suggests severe infection, can be dangerous and often need to be in hospial. Fever should trend lower and lower every day, spikes, but less and less. If worsens, changes, or fails to improve, need to seek immediate medical attention.
