See MD: If there are signs of respiratory distress. A respiratory rate of 21 is ok for a 2 y.o., but if he is "gurgling" or having difficulty breathing he should be evaluated by a Dr. If he is lethargic, not feeding, poor color, nostrils flaring, and/or you can see his ribs retracting with each breath, then he needs prompt evaluation.
