What's he wearing : I would wonder about a chronic low grade contact allergy. Footwear related dyes, latex in socks, etc. , poorly ventilated shoe ware over hydrating the area. That's a head scratchier for sure.

Possible Reynauds: Have him checked for what's called reynauds. Quite common in fact I have it.
