Maybe backwards: Sometimes, a patient gets brain damage, and the damaged brain is not working correctly (thus leaves the patient in a coma). The damaged brain swells, and can be seen on a scan as "brain swelling". The patient may die from the original brain damage, the brain swelling that was due to that brain damage, or die from other causes. One has to look at the whole picture to see what might be happening.
