Physical exam: I agree with dr. Singer. There are certain aspects of the neurological exam that can also be done which can help determine if the brainstem is functioning. But the confirmatory tests are outlined by dr. Singer.

Neuro exam ...: To determine brain death, a thorough neuro exam is performed. If no function is evident, a apnea test is performed to ensure that no respiratory drive is present. If none with a pco2 of >60, then patient is declared brain dead. There are a few more details, but not enough room here to add.
