B12 Deficiency: There are several things that happen with a B12 deficiency including anemia, neuropathies, and non-soecific neurological affects such as fatigue, depression, irritability, and psychosis. Clinically, patients are diagnosed with B12 deficiency through anemia first but with current testing practices many patients are discovered to have low levels without ever being anemic.

Sometimes: While low B12 is a cause of one type of anemia low B12 is associated with many other things but levels are based on "statistical norms" so slightly low levels shouldn't cause symptoms yet but if you and i both have the same low level one of us might be anemic and the other not! it just happens that the most common way to discover low B12 is through a particular pattern of anemia.

Not necessarily: Low B12 can manifest w/o the development of anemia initially. Supplementation with either injections or with oral supplements may be in order if the B12 level is documented to be low by blood test.
