Same cost: The price of dental procedures will cost the same amout whether you have insurance or not as dentists are not allowed to have separate fees for the same procedure. Talk to your dentist and get a second or third opinion, but the teeth should be straightened so the bite, muscles and upper quadrant function in harmony.

Depends: The complexity of your case determines the fee. Since each office charges differently for services, your best bet would be to consult with a local orthodontist who can actually see you, diagnose you, and treat you. No one over the internet can do that.

Please see a fully qualified Orthodontic Specialist, someone who does only Orthodontic Care, for an initial examination (usually free). The Orthodontist can competently answer your questions once he/she has examined you.

Every dentist is dif: this depends on the area and how complex the treatment will be. Unfortunately there is not much of a discount on treating only one jaw. Ask at your consults
