Rule out lupus, FMF: There are a few causes of serositis that might do this including lupus and familial mediterranean fever. I trust you have had really good imaging studies done and do not have meig's or a portal vein thrombosis. This is a difficult workup; good luck.

Needs an exam: Should see a gastroenterologist immediately. Causes can be worked up by a detailed workup without any further delay and liver compromise. Might need a liver biopsy if warranted by your physician.
