See a specialist: Your questions cannot be adequately answered without a thorough exam and evaluation and x-rays. I would see a dentist who is trained to deal with tmj/tmd, or a tmj/tmd specialist.

TMJ: Almost assuredly TMJ. The average of onset of temporal arteritis is 55. See a TMJ expert. Any dentist can be a TMJ expert with the proper training and experience. Most commonly, oral surgeons, prosthodontists, and orofacial pain specialists. Ask your MD, your dentist and your dental society for referrals.
