Had patient with low testosterone and mri, showed 2 small pituitary lesions 4mm and 2mm, when should i do f/u mri . is 2 lesions not one significant?

Family practice: Pituitary adenomas are present in 1:1000 of the general population. Most are small and benign. I would get another in 6 months but you need to se a neuro surgeon to follow you.

Evaluation: Was the prolactin high. Is that why the testosterone is low? I would work this up and make sure it is not a functioning tumor. If non functioning, i would get another MRI in 6-12 months.

Functional adenomas?: I presume this is an older man? Mris are so sensitive, they detect these tiny pituitary lesions, which sometimes are not functional or significant. Agree with checking prolactin. If that is normal, and if thyroid/adrenal axis is ok, you may wish to follow him, treat low t as you see fit. Doubt this is surgical. A repeat MRI can be done in 6-12 mos, but i suspect it will look the same.
