BPH PSA : Normal psa is usually below 4.0 prostate enlarges with age, so psa is proporionate to prostste size. So, psa of 8.0 might be normal for 77 years old. 'dis-proportionate' psa, way too high, can mean prostate cancer. Fyi: as per current protocol & guidelines, we do not do psa any more. Or we are not supposed to. It is not sensitive or specefic. 'just don't do it'.

Less than 8.0ng/ml: The age-adjusted scale below often is used to determine if your psa level is within normal range. Talk to your doctor about what your results mean for you. Age normal psa range <40 0 to 2.0 45 0 to 2.4 50 0 to 2.8 55 0 to 3.3 60 0 to 3.8 65 0 to 4.5 70 0 to 5.3 75 0 to 6.2 >80 0 to 7.2.

Psa: I think health status is more important. If the 77 looks good and is very healthy i would consider a bx at a lower value. In an unhealthy 77 yo, i would hold off doing a bx unless the psa was very high. Psa should not be done on a pt who has less than a ten year life expectancy. Over 4 in a 77 yo old should prompt a bx discussion.
