Depends, but...: Most scar revisions can be carried out with local anesthesia, sometimes in the office (rather than an operating room), but it depends on the size and nature of the scar as well as the personal preference of the surgeon and patient.

Depends on You: If your pain and anxiety threshold are manageable with some coaching and local numbing medication, a simple appendectomy scar revision is doable in the office. If your highly anxious about the procedure, you may be better served with IV sedation by an anesthesiologist.

Local: Throughout the years of doing scar revision i believe that the patient is most comfortable using sedation with oral medications and local medications to revise scar.

Local if cooperative: If the idea of having the revision perfromed under local sounds ok to you, it will probably be ok with your surgeon too. If you don't like the idea, or can't sit still, then general anesthesia can be used.
