Sometimes not: Punctal plugs have a long and relatively safe track record. But they are not for every one. Frequently they are not used appropriately. While supposed to be inert. People can have allergic reactions to them and they can get infected. They are part of a complex set of treatments that should be individualized.

When used properly: When used for the proper patients they are perfectly safe. I would not class them as aggressive at all. If you would prefer not to use them, there are other treatment options.
