Getting history: In order for a psychologist or psychiatrist to provide the most optimal treatment for you, you should fill out forms about your problems as the way you see them. You're not obligated to fill out any form (i have many patients who declined to do so, but i still see them). It's in your best interest to be honest - psychiatric records are more confidential, and it'll help max your treatment.

Varies A great deal: These vary from simple demographic and insurance forms to more lengthy forms such as ours which exceeds 18 pages. You may get a hipaa form, release of information forms, and a practice or insurance payment agreement form. Forms may differ for different ages, purposes and the specialization of the psychologist. We do a lot of health psychology and neuropsych. Even my md patients complain! : ).

Psychology Forms: I have an online form that patients fill out before their first visit detailing some of their concerns and history. I also have a consent for giving legal permission for me to contact the insurance for billing purposes. It also describes my payment policy and confidentiality restrictions.
