Varies: If its a large defect, it's mostly an unsightly bulge and a loss of strength. If the defect is smaller, risks include incarceration or strangulation of bowel loops.

Could get worse: It could get larger, or cause symptoms such as pain. Also, whatever is coming through the hernia (typically fat or intestines) could become stuck and require emergency surgery to save your life. This is unlikely, but unpredictable.
