Endo and NS: An endocrinologist has to determine if the adenoma is functional. If a prolactinoma, even if large, medication is started before considering surgery. Other pituitary hormones have to be assessed as well. Cortisol and thyroid hormone deficiencies have to be replaced before surgery. Hydrocortisone is usually administered prophylactic ally if surgery is necessary. Size and symptoms important also.
