Pockets deeper 6mm: When the space between teeth and gums are deeper then 6 mm and if inflammation is not reduced by deep cleaning is not resolved periodontal surgery may be required. Can not explain in a small time. Ask your dentist.

Pocket reduction: The goal of any periodontal surgery is the reduction of pockets around affected teeth to patient maintainable depths (less than 5mm). Pockets deeper than 5mm are not cleanable by a patient at home. Bacteria and plaque will continue to propagate in the deep pockets causing further infection and bone loss. This is accomplished by cutting away diseased tissue around the teeth and reducing the pockets.

Prevent tooth loss: Periodontal surgery to treat "gum disease" usually involves making small incisions and lifting the gums from the teeth to access the roots and remove infection. After that, the gums are sutured back into place. Typically, most patients will take Ibuprofen for a few days afterward and be placed on a prescription mouthrinse. There are other periodontal surgeries not discussed here.
