Renal dosages....: Pradaxa is a strong blood thinner (direct thrombin inhibitor); needs to be taken carefully to Kidney patients to prevent BAD outcomes. If Crealtinine clearance 15-30 cc/min dose rec is 75 mg bid (twice daily). So 150 mg daily dose is odd. SECOND, if creatinince clearances
