Otologist: If you've seen an ENT he has done tests and given your a diagnosis/treatment. If you want another opinion, I'd recommend an otologist. A ENT expert with a additional fellowship in ears.

Sometimes my ear drum thumps/spasms when I'm talking. I'm a bilateral tinnitus and tmj sufferer. Hearing is good, normal exams from ENT. I do get pain and fullness in my ear when I'm having a day like that at times, but sometimes just thumps no pain?
