Newer MS lesions: tend to contrast enhance. A detailed knowledge of neuroanatomy is needed to correlate the relationship between MS lesions and specific symptoms. High powered MRI (> 3.0 tesla) can detect additional lesions in the cerebral cortex that can otherwise escape detection with conventional MRI. (White matter lesions show up well on standard 1.5 and 3.0 tesla machines) Spinal cord lesions need 3.0 or >)

Comments: MRI films can show acute T-1, gadolinium positive lesions which are new and are present for about 3-4 weeks. The T-2 lesions represent older areas of inflammation, but may change in size and number over a period of weeks to months. T-1 black holes are older lesions, often c/w permanent areas of damage. Need to base treatment decisions on lesion characteristics.
